Typically, if the stylus is a 5-way we would define the furthest tip as having an overall length of say 144mm with 0,0,0 offsets. The remaining tips would be defined as having an overall length to the height at which the 4 stylii are located from the centre of the PH head and then use the xyz offsets to locate the tips in the xy axes.
If you have CAMIO, try loading in a part model then the P144d25s.sat probe file. This models a 5 way stylus. Issue a SNSDEF, then pick a stylus tip. This will write the correct values for you.
I think your main problem may be the sign values for the xy values because these indicate where the tip is pointing so we can calibrate it.
